Whirlpool Duet GHW9250 Pulley problems

I have a Whirlpool Duet GHW9250.  I have replaced the pulley 3 times in the last 5 years, the last time only 5 months ago, each time because the belt came off and when I inspected it, the pulley was wobbly on its shaft.  The same thing happened today.  I can replace the pulley again, but I'm concerned that some other problem is causing the pulley to wear prematurely.  Any suggestions about what I should look for?

I've never had that failure. How is the tub bearing free play? Does it get pretty loud during spin portion of the cycle?

A good way to easily check the bearing play is to reach inside the tub and try to move it up and down or side to side. If you notice movement beyond the shocks you have bad  tub bearings. I have run into this problem before. it usually coincides with broken shocks. Check your shocks too.

I've checked, and there doesn't seem to be any problem with the bearings.  I can't move the inner tub independent of the outer tub.  However, I have another thought, which I'll put in a different post because it isn't related to the pulley at all.

You are correct in thinkiing it could be something other than the pulley. Is the shaft perfectly straight? Is the washer level and legs locked into place with lock nuts? Is the floor solid? Sometimes second story floors and some wood floors do not offer the support needed for high spin speeds. Unbalanced loads can cause issued like this too. don't forget to tighten belt correctly, do not over-tighten.

The washer is level with legs locked into place and is in the basement on a concrete slab.  The shaft is straight, and the belt is properly tightened.

 

I noticed one issue, though. I replaced the motor two years ago, and I first had to replace the pulley about 9 months later.  While I was looking at it today, I noticed that motor shaft is a bit too short.  This makes the path of the belt slightly askew as it wraps around the pulley and then over the motor shaft.  It's not off by much, maybe 3/4 of an inch, but I'm betting that would cause the pulley to wobble and wear at the point where it attaches to the drum shaft.

 

My question is: did I somehow install the motor wrong or is there something wrong with the motor I purchased.
