Need help diagnosing AC condenser fan issue and help finding a replacement motor

hi,

My AC condenser is coming up on 30 years old.  It cools the house pretty good for a unit that age but just this weekend that condenser fan stopped working.  It had been making a different sound this week so I knew something was up.  
 

Today I tested the capacitor which is a 3 year old Turbo 200 as well as the fan motor.  while the unit was running fan was drawing 1.2 amps

After performing my initial tests everything actually started up and was running for about 45 minutes.  

After shutting down the unit to close up the cabinet the fan wouldn’t come back on.  I rechecked the capacitor and fan motor and the resistance on the motor was much higher than when the motor was cold.  The amp draw was 3. After a quick assist of spinning the fan blade by hand it started right up and died 20 minutes later.

I need some help confirming my hunch that the fan motor is bad and also need help finding a replacement motor because the part numbers that come up in search are different.

I’m will try to attach some photos that show the fan motor and capacitor wiring.

The AC model is 38Tkb048300

The fan part numbers are 5kcp39egP611s / HC39GE234A, 1100 rpm, 1.4 A

My test results for the capacitor from common to the various options are As follows:  expected mfd (actual mfd)

25 (23.55)

20 (19.83)

10 (9.92)

2.5 (2.547)

5 (4.67)

5 (5.66)

And the fan motor ohm readings cold

Yellow to Brown 83.2

Yellow to Black 26.1

Black to Brown 57.2

these numbers were somewhat higher after the first failure today but the two lower numbers equaled the higher one.
